This document is a request form for an employee to have their salary and allowances paid directly into their bank account. The employee provides their signature, rank and date and requests that all current and future sums of money due to them be paid to the specified branch of their bank for credit to their account. The bank's acknowledgement of the payments will serve as full discharge of the obligation. This request cancels any prior payment authorities.
