SUBSCRIPTION CONTRACT
KNOW ALL PERSONS BY THESE PRESENTS:
This subscription contract is made and entered this ______ day of _________ by and

W I T N E S S E T H : THAT-
WHEREAS, the Corporation has an authorized capital stock of Fifteen Million
Pesos (P15,000,000.00).
WHEREAS, on ______________ the Board of directors and stockholders of the
Corporation have approved the increase of its authorized capital stock to One Hundred
Fifty Million Pesos (P150,000,000.00) divided into Fifteen Million (15,000,000) shares, with
a par value of Ten Pesos (P10.00), per share.
WHEREAS, the corporation has offered to sell shares of stock from the said increase
of capital and the Subscriber is willing and ready to buy the same.
NOW THEREFORE, in consideration of the foregoing and the subscription price
indicated hereunder:
1. The Corporation has issued to the Subscriber the following shares of stock:

2. Of the subscription price, the Subscriber has paid to the Corporation the amount of
