Attached hereto is a copy of his Certificate of Live Birth from the
PSA marked as Annex “A”;

2. My son, ________________ is applying as a member of the


Philippine Army. Thus, I hereby give my full support and consent
to her application;

3. I am neither forced, coerced nor intimidated in giving this consent;

4. I am executing this affidavit to attest the veracity of the foregoing


and for purposes of extending my consent to
