Nursing Care for Anxiety & Activity Intolerance

The patient reported moderate anxiety related to an upcoming surgery as evidenced by an increased blood pressure. The nursing diagnosis was moderate anxiety and the objectives were for the patient to report decreased fear, anxiety, and blood pressure as well as feeling more relaxed within 2 hours of interventions. Interventions included establishing rapport, explaining the procedure, promoting deep breathing, and administering antihypertensive drugs. After 2 hours the goals were met as the patient appeared more relaxed.
