AFFIDAVIT OF LOSS
I, SHEILA MARIZ QUISEL TUBAC, a Filipino of legal age, married, and a resident of
with postal address at Tapon Norte, San Jose, Negros Oriental, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state:
1. That having satisfied the requirements of the different government office, I was
issued identification cards for my Philippine Health Insurance Corporation, and LTO Driver’s
License;
2. That having also satisfied the requirements of RCBC, BDO, and PNB, I was issued
a RCBC ATM Card, a BDO ATM Card, and a PNB ATM Card;
3. To my surprise, I discovered last February 24, 2020 that the said Identification
Cards could not be found;
4. That all efforts to find the same proved unrewarding, it is now safe to conclude that
recovery of the same is an impossibility;
5. That I am executing this affidavit to establish the loss and for the issuance of new
identification cards;
6. That this affidavit is further executed for whatever legal purpose this may serve
best.
