After School Club Parental Permission Form

Carver Christian Fellowship


PARENTAL PERMISSION FORM

Dear Parent/guardian,

English teacher, Mr. Tobass, is hosting Carver Christian Fellowship, an after-school club where students
can come to play board games, enjoy refreshments, spend time with one-another, and have open
discussions on spirituality and religion, specifically Christianity. This program will run on Tuesdays
from 2:45 – 4:15. There will hopefully be additional field trips that take place. In the event of additional
trips, separate permission slips will be sent out. Students can be picked up any time before this, or can
stay after if given permission by Mr. Tobass as well as parents and guardians of the individual students.
All are welcome! It is a safe space where all voices are heard, and all can share.
