Date/Time Order Rationale

The doctor ordered the patient to be admitted to provide necessary care. Vital signs were to be taken every 2 hours and blood pressure every hour to monitor treatment. The patient was placed on a low salt, low fat diet to control cholesterol, triglycerides, blood pressure, and fluid retention. Intravenous fluids and various labs including a urinalysis, CBC, electrolytes, ECG, chest x-ray, and liver and kidney function tests were ordered to evaluate the patient's condition. Medications including hydralazine, amlodipine, losartan, and metoprolol were prescribed to treat hypertension. The doctor continued monitoring the patient and adjusting medications to improve their condition.
