Dear Sir:

One of the requirements of the Commission on Higher Education (CHED) for students
taking Bachelor of Science in Accountancy (BSA) is an On-The-Job-Training (OJT) related to
their academic preparation in the area of accounting or auditing. The training will be for a
minimum of 200 hours.

On this regard, we would like to request that Angelica Joy G. Castillo, 4th year BSA
students of this institution be allowed to undergo their OJT in your prestigious company. Should
this request be granted, the student’s availability is from Monday to Thursday, 8:00 AM to 5:00
PM and Friday 8:00 AM to 12 PM to start upon your confirmation and until such time they will
have completed the required number of hours.

Attached for your review and or comments are the following documents: Intern’s
Resume, Memorandum of Agreement, Confidentiality Agreement and Waiver/Parents’ Consent.
May we request you to go over these documents. Should you have a query on any of the
provisions, please feel free to raise them up with the undersigned. Should there be none, we
would highly appreciate if you can affix your signature to signal your willingness to accept our
students.
