RELEASE WAIVER AND QUITCLAIM
KNOW ALL MEN BY THESE HERE PRESENTS:
1. ________________________________ of legal age, with address
at __________________________________ hereby acknowledge and
accept the lawful termination of my employment with LARESIO
LAKESIDE RESORT & SPA INC., (“The Company”, for brevity),
as a result of retrenchment, in accordance to existing labor
rules and regulations, and by these presents manifest and
declare that:
1. I have and been paid the amount of Php __________________
representing the full and complete settlement of my monetary
claims. I further declare that I have been paid in full amounts
representing my salaries, allowances and other benefits,
privileges, monetary or otherwise due me from the Company,
its owners, its agents and/or employees whether such amounts
are due me by virtue of any law, company program, and/or
company practices.
2. In consideration of the foregoing payments, I hereby
acknowledge and accept the lawful separation of my
employment with the Company and hereby release, waive and
forever discharge the Company, its owners, agents and/or its
employees from any action, whether civil, administrative,
criminal or otherwise, or from any claim of any kind or
character on account of the termination and cessation of my
employment with the Company, including, but not limited to,
salaries, allowances, holiday pay, fringe benefits, separation
and retirement.
3. I will not, at any time and any manner whatsoever, directly
or indirectly, disclose to any person or entity any knowledge or
information of any kind and character relating to the business
or operation of the Company, including but not limited to, any
of its clients, its manner of operation, plans, processes,
practices, programs, corporate schemes or data of any kind or
character which I acquired, learned or received during my
employment. I bind myself to indemnify the Company for all
damages that it may suffer of will suffer as a result of my
violation of this obligation.
4. I unconditionally, obligated myself to turned over and
accounted all company records, properties, and assets in my
possession not later than the date of effectivity of my
separation or upon demand of the Company and prior to my
departure from its premises. I further undertook to render
every cooperation and assistance, which the company requires
for said turnover and accounting.
5. I have no cause of action whatsoever against the Company,
company owners, its officers or employees with respect to any
matter incidental to or arising out of my employment with the
Company due to the cessation of my employment.
In as much as I have no cause of action against the company,
its owners, agents and employees, I will never at anytime
initiate file or pursue and claim or action against the company,
its owners, agents and employees for any and all matter/s that
arose, could have arisen, in connection to my employment with
said company, the termination of my employment therein and
any other monetary claims in relation therewith.
6. I hereby release, waive and hold harmless the company, its
owners, agents and employees for any and all action or claim
involving any and all matter/s that arose, could have arisen, in
connection to my employment with said company, the
termination of my employment therein and any other monetary
claims in relation therewith.
7. This Release, Waiver and Quitclaim is the complete and
exclusive statement of the agreement between me and the
company and supersedes all proposals and prior agreements,
oral or written, and all other communications between the
parties relating to the subject matter of this release.
8. I have executed this Release, Waiver and Quitclaim
voluntarily and without mental reservation and only after
honest and full reflection on all the circumstances pertaining
to the incident. For this reason, I bind myself to defend the
voluntariness and truthfulness of this Release, Waiver and
Quitclaim, before any forum and under any circumstance
which my personal participation may be needed.
9. I manifest that the grant to me of the amount herein before
stated shall not be taken by me, my heirs, or assigners as a
confession or admission of liability on the part of the Company,
company owners, its officers or employees for any matter,
cause, demand or claim of damage.
10. I finally, declare that I have read this entire document, the
contents of which have been explained to me and which I
acknowledge to have signed, and this Release, Waiver and
Quitclaim hereby given is made by me willingly, voluntarily and
with full knowledge of my rights and is binding upon me and
upon my heirs, successors-in-interests and assigns.

nauunawaan ang mga taning at pasubali ng dokumento na ito,
maliwanag ang lahat na naipaliwanag sa akin at ito ay
pinirmahan at tinanggap ko ng bukal sa aking kalooban. Sa
katunayan, lubos ang aking pasasalamat sa kumpanya, sa
mga may-ari at mga kapwa ko empleyado.”
