Patrick Prado Bautista owns two dump trucks that were confiscated by DENR Balingasag for failing to provide delivery receipts for loaded sand. He had rented the trucks to AH Trucker Construction Aggregates and Trading and entrusted the operation to drivers. SWORN AFFIDAVIT

I, PATRICK PRADO BAUTISTA  of legal age, Filipino, presently resided at


Camaman-an, Cagayan de Oro, Misamis Oriental, after having been duly sworn to
in accordance with law, depose and state:

1. That I am the Owner of the two (2) Units Dump trucks with LTO CR Nos. 36223209-1
and 362223207-6; with Plate Nos. NEG1948 and NEG2065;

2. That my two (2) Units Dump trucks were hired and rented by AH Trucker Construction
Aggregates and Trading;

3. That on March 07, 2020 the said Dump trucks have been held by DENR Balingasag due
to failure in showing the Delivery Receipts of the loaded sands;

4. That I entrusted the said units to my drivers including the operation, deliveries, and
purchases;

5. That I have been unaware of my Drivers’ negligence, and for being so very
irresponsible;

6. That those Drivers haven’t reported to me personally the incident since then, instead
have left the company without consent and proper notification at me even at the
company’s supervisors or somebody and/or those drivers are in AWOL status;

7. That I affirm and declare that I am the owner of the two (2) Units confiscated Dump
trucks;

8. That I am executing this affidavit to attest the truth of the foregoing facts and for
whatever purpose this may serve.
