Dear Sir/Ma’am,

Good day!

I am relieved when your department has recently announced the financial assistance
program for private employees who are directly affected by this pandemic crisis. Please
know that your initiative has brought a good portion of hope to my employees at TMS
Lodge, Cebu City. I feel so much for these folks because I am a living witness of how
much they are striving just to earn a living; and yet, now, due to the strictly-imposed
home/community quarantine, there is no way for them to feed and support their
struggling families.

In this connection, on behalf of my valued employees, I am writing to formally appeal


and apply for the COVID-19 Adjustment Program (CAMP) 5K-Financial Assistance. I have
at least _____ number of employees who are sure qualifiers for the program. It will be of
huge help for them to be approved of the CAMP grant. You have no idea how much this
means to us. In this time of challenge, humanity only have each other, and there is no
other way to survive but to help each other.

Thank you for the time and consideration you have given our situation. I will be
expecting for a positive response to my humble request. Attached herewith are the
other documents you requested for approval.
