program. The program highlights important points, but the test questions will be derived from the
entire document. (co-workers may share study materials, but must complete the online certification

3. After receiving your registration, STI will e-mail to you
A web link of the test
User Name and Password.
Using these, you will be able to pay for the use of the program by credit card and then
begin the program immediately.
You will then have 48 hours to complete the program (after 48 hours, the program will no
longer be accessible by you).
The program consists of 11 modules with a PowerPoint presentation followed by 4 quiz
questions. After answering at least 3 of these quiz questions correctly, you will be able to
continue to the next module. The modules must be taken in the order that they appear.
The final test consists of 30 questions and you must achieve a score of 80% to pass and
obtain the certification.
4. The initial cost of this program will be $250. In exchange for this reduced rate, we ask that you
provide feedback on the program and understand that as is true with any new web-based program,
there may be some small glitches at the beginning. Your patience will be appreciated.
5. Certification of passing an eye exam (visual acuity exam) must be Fax or mail registration to:
sent to STI prior to the issuance of STI SP001 API 653 Adjunct STI/SFPA
certificates. The eye exam must be administered by an optometrist, Attn: Deb Timmerman
