Re: MARCH AND APRIL 2020 NOTARIAL REPORT

This has reference to the mandatory filing of monthly notarial reports imposed upon
commissioned notary public(s) pursuant to 2004 Rules on Notarial Practice. However, due to the
COVID19 pandemic and the Enhanced Community Quarantine within the jurisdiction of the
Honorable Court the undersigned notary public belatedly filed this notarial report.

In this regard, please acknowledge the receipt of the March and April 2020 Notarial Report
of the undersigned Notary Public for and in the City of Manila under the appointment made by
the Executive Judge of the Regional Trial Court-Manila on ___ 2020 and expiring on December
31, 20__.

Attached herein are the photocopies of the loose leaves of the notarial register and copies
of the corresponding notarized documents, to wit:
