AFFIDAVIT OF LOSS

I, _____________, of legal age, single, Filipino and a resident of


____________________ after having been duly sworn according to law, depose
and state:

1. That I was issued a student licensed driver in accordance with pertinent


Land Transportation laws, rules and regulations, however the date of
issuance and registration number, I can no longer recall;
2. That sometime on 2014, I was apprehended in a check point by the duly
constituted authorities along ____________ and a Temporary Operator’s
Permit (TOP) was issued to me bearing the number _________________;
3. That sometime in the same year, I noted that said TOP is missing;
4. To my knowledge, said TOP was lost when I moved my personal
belongings to my current residence;
5. That despite diligent search and efforts to locate my TOP, I could not find
the same, such that I now believe that it is now lost beyond recovery;
6. That said TOP was not confiscated by the LTO, Police, other Traffic
Enforcers, for any violation and/or infraction.
7. As such, I am executing this Affidavit of Loss to attest to the truth of the
foregoing and for all legal intents and purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 12th day of March
