AFFIDAVIT
I, JENNIFER L. APANITAN, of legal age, widow, Filipino, and a
resident of Barangay Gabu-an, Roxas City, under oath, depose and say:

1. I have four (4) dependents/children and one of whom is


JERRICO LABAROSA APANITAN who is applying for
Scholarship with the “Tulong Dunong ni Cong Tawi Billones
Program”;

2. I am only a laborer and already a widow and I have not filed any
Income Tax Return because I have no fixed income, and my
income does not exceed Ten Thousand (P10,000.00) Pesos in a
year.

3. I do not possess any income generating property;

4. I voluntarily execute this Affidavit to attest to the truth of the


above-mentioned facts.

AFFIANT FURTHER SAYETH NAUGHT.

IN WITNESS WHEREOF, I have hereunto affixed my signature on
