residing at Poblacion, Kidapawan City, North Cotabato, after having been duly sworn
in accordance with law, do hereby depose and say;

1. That I am Grandmother and guardian of ALTHEA BAYAWANON MAWAY


born on February 15, 2014;
2. That the above-mentioned name is under my care and custody;
3. That I am not imbecile, insane, a vagrant and vicious person or a habitual
drunkard or habitual criminal, and have not abandoned, neglected or refuse to
support said minor or caused her to commit offenses against the law;
