Citizens Affairs the following money Three Thousand pesos only (3,000 php). I am not able to
represent myself because of my disability and I will not be able to meet the deadline.
I hereby authorize Lucila Silva Delima to act on my behalf and perform all the necessary
transactions required to complete the process. Lucila Silva Delima shall be presenting his or her
Identification card or passport for verification. I also permit Lucila Silva Delima to receive the
money. I understand that Lucila Silva Delima will not be responsible for any loss of documents
or delays incurred during the processing procedure and delivery.
Lucila Silva Delima is not allowed to use the received money for any other purpose not
mentioned in the letter and she should not also be allowed to make any other decisions on my
behalf without written consent from the Office of the Senior Citizens Affairs. Please accept my
letter of authorization as a confirmation that I grant permission Lucila Silva Delima to claim my
Financial Assistance from the Office of the Senior Citizens Affairs
