AFFIDAVIT OF LOSS

I, KENNETH JOHN B. FERNANDEZ, of legal age, Filipino, married and a


resident of Block 1, Lot 31a, Parc Regency B, Pavia Iloilo, after having been duly sworn
to in accordance with law, hereby depose and say:

1. That I am the owner of an original copy of certificate of transfer and original


copy of transcript of records.
2. That the said documents were duly kept in my document files.
3. That upon inspection, the said documents were nowhere to be found.
4. That the said documents were lost for all intents and purposes and cannot be
located despite all efforts exerted to locate the same.
5. That I execute this affidavit in order to request for a new original copy of the
said documents..
6. That I am executing this affidavit to attest to the truth to all the above facts.
