AFFIDAVIT OF DESISTANCE

I, _________, of legal age, Filipino, married, a resident of _________, City


after having been duly sworn to in accordance with law, hereby depose and state:

1. I am the private complainant in NPS Docket No. _________for


Perjury penalized under _________of the Revised Penal Code against
_________before the Cebu City Prosecutor.

2. After a careful review of the facts, I have come to realize that the
matter involving the subject of the complaint for _________was brought about by
conflicting claims which me and some of my children including _________ on the
_________could have been averted and settled amicably between and among
family members.

3. In pursuit of _________, in which I have settled the conflicting claims


involving my _________, the family is likewise working towards the same
objective of settling the family dispute with my children. Consequently, I have
decided not to pursue any action, civil and criminal, against , on any matter
