Dear Ms. Dianne,

I am writing this letter to formally resign from my position as a Intensive Care Nurse at
Healthserv Los Baños Medical Center. I am issuing this notice of resignation 2 weeks in advance
of my final day of work, which will be on August 15, 2020.

During my 2 years at this hospital, I have learned and enhanced my skills in collaborating with
different types of individuals to ensure that the high quality of patient care is delivered. As our
slogan states “Patient First”. I am very grateful to have worked with such committed and
dedicated people who shared me too much knowledge that mold me as to who I am today.
People who share my desire of giving the best quality to work to our patient.

Thank you for all the opportunities you have provided me during the last 2 years of my stay. I
wish my colleagues and every staff of Healthserv more success in the future and will be safe
and protected as we face the COViD-19 pandemic.
