CONTRACT OF SERVICE

AS QUARANTINE FACILITY FOR STRANDED OVERSEAS CONTRACT WORKERS (OFWs)


DUE TO THE COVID-19 PANDEMIC

This Contract is entered into by and between:

The, a local government unit duly created and existing by virtue of Philippine laws, with
principal office address at the s, hereinafter referred as the CLIENT, represented in this
instrument by the per SP Resolution (Copy attached); and

___, a corporation duly created and existing by virtue of Philippine laws, with business
address at, hereinafter called the SERVICE PROVIDER, represented by its ___, ___;

WITNESSETH:

WHEREAS, the Philippines has been declared under a public health emergency due to
the COVID-19 pandemic, and the has imposed quarantine measures to prevent the spread of the
virus to the;

WHEREAS, there are Overseas Foreign Workers (OFWs) who are residents of who have
been stranded in Cebu City and Metro Manila who are scheduled to travel back to the;

WHEREAS, while the said stranded OFWs have completed their mandatory 14-day
quarantine in Cebu City and Metro Manila, they still need to be quarantined for another 14-day
period before they will be allowed to go back to their respective families in compliance with
local orders and pursuant to the agreement of the and the League of Municipalities of the
Philippines- per Resolution with Minutes herein attached;

WHEREAS, the SERVICE PROVIDER has been accredited as qualified to be a


Quarantine Facility in the;

WHEREFORE, premises considered, both parties agree on the following:

1. The SERVICE PROVIDER agrees to render its establishment as quarantine facility for
the fourteen (14) – day quarantine period of the following OFWs who are residents of
Tagbilaran City, Bohol, to wit:

2. The quarantine for the said OFWs shall cover the period from May ___, 2020 to May
___, 2020;

3. The total contract cost for the accommodation of the quarantined of the said OFWs shall
be in the amount of __________ Pesos (P_____.00), at the rate of P1,000.00/person/day;

4. The Service Provider agrees to comply with the technical requirements of designated
Health personnel who will oversee the 14 – day quarantine of the OFWs;

5. The total contract cost of the accommodation shall be paid after the end of the 14 – day
quarantine period;

6. The agreed total contract cost shall include the use of the designated premises by health
personnel for the whole duration of the period;
