AFFIDAVIT OF LOSS

We, MICHELLE LIZA ODSIGUE CO, of legal age, Filipino, and resident of
U-1122 Manansala Condo, Rockwell, Makati City after having been duly
sworn to in accordance with law, hereby depose and state:

1. That I am the Managing Director of PEG Social Media Inc.;

2. That, as Managing Director several contracts of our clients pass


through my office;

3. That on March 17, 2020, while in my office in Worldplaza, BGC, I failed


to find the contract between Love Marie Ongpauco-Escudero a.k.a
Heart Evangelista and Multiflex RNC Philippines, Inc.;

4. Despite diligent search and efforts to locate the said contract, I could not find the
same such that I now believe that it is now lost beyond recovery;

5. That I am executing this affidavit to attest to the veracity of the


foregoing facts and for whatever legal purpose it may serve under the
law.
