KNOW ALL MEN BY THESE PRESENTS:

THAT WE, PERSEUS C. GA JR., CHRISTINE MARIE C. GA, JESSICA O. GA and


ANGELIE C. CASISON, all of legal age, Filipino citizen, and resident of Zone 2, Brgy Kisolon,
Bukidnon, do hereby name, appoint, and constitute JONATHAN C. GA, likewise of legal age,
Filipino citizen and a resident of Zone 2, Brgy Kisolon, Bukidnon, to be our true and lawful
attorney-in-fact, for me and in my name, place and stead,
to do and perform the following:

To apply and process the Social Pension for Indigent Senior Citizens


named after our late father Salvador Perseus G. Ga under the
establishment of Department of Social Welfare and Development; (attached
herein is the death certificate of our father)

To receive the benefit of Social Pension for Indigent Senior Citizen; and

To sign, submit and receive all documents and papers related thereof under
such terms and conditions as our attorney-in-fact may deem proper.

HEREBY giving and granting unto our said attorney-in-fact full power and authority to do
and perform any and every act and thing whatsoever requisite, necessary or proper to be done in
and about the premises as fully to all intents and purposes as I might or could do if personally
present and acting in person; and

HEREBY ratifying and confirming all that our said attorney-in-fact shall lawfully do and
cause to be done under and by virtue of these presents.
