which I have read all the content hereof and fully understood the same when translated
before me in vernacular;

5. With this, thus, I hereby declare and acting without any malice that my civil status is still
SINGLE for I have not entered into marriage before any religious or civil rituals or
practices with any individuals;

6. I am executing this affidavit to attest as to the veracity of the foregoing facts particularly to
inform the Philippine Army (PA) of the Armed Forces of the Philippines (AFP) relative
to my application for membership service under the said military forces and for whatever
legal purpose/s this may serve best.
