S No CORRECTIVE ACTIONS TAKEN/IMPLEMENTED
  1 Security concern:
The walkthrough revealed that there was a concern for security, as there was no
restriction on the personnel being able to enter the hoarding at all times. The suggested
action was to appoint a dedicated security staff in order to control the movement.
          Action taken:
A staff was appointed to constantly monitor the movement into the hoarding therefore
reducing the risk of security problems.
  2 Safety Signage:
The signage and the posters that were put up at the site were ambiguous and not clear.
It was not giving the desired impact and needed to be replaced by better and more
          accurate signage.
    Action taken:
The signage were changed with much higher clarity and relevance to the site was
    assured.
3 Safety induction :
The visitors were not inducted upon entry into the hoarding.
    Action taken:
The safety induction for visitors is attached and will be followed rigorously.
4 Safety Notice Board:
The size demanded a safety notice board for employee awareness and training.
    Action taken:
The board was brought into the site and the safety notices, HSE stats, emergency
contacts and the evacuation plan was displayed on the notice board
5 Emergency Evacuation Plan:
No evacuation plan was on display. All the personnel on-site should be above the
evacuation plan which should be on site indicating all the nearest exits
    Action taken:
Evacuation plan was prepared and was displayed on site.
6 Trip hazard:
The site had protruding nuts as a leftover of the spider post fittings. These nuts were
exposed to everyone and would easily induce a trip hazard. The area was suggested to be
    barricaded off immediately
    Action taken:
The area was immediately barricaded off. Eventually the work was completed and the
trip hazard was eliminated from the root.
7 The materials shifting (Sky Train):
The materials shifting was don through the passenger sky train, so there were potential
issues with the safety of the passengers
    Action taken:
    Materials Shifting plan is attached.
