This is a survey for the grade 7 students of Caraga Regional SHS. This questionnaire is intended to measure the
factors affecting the English proficiency of a student, which means ‘a high degree of skill’, in such areas of personal,
family, school, environment (social and physical, and mass media. Please answer accordingly.
