AFFIDAVIT OF LOSS

I, ___________________, of legal age, married, and a resident


of______________________, after having duly sworn to in accordance with
law, do hereby depose and state:

1. That I was duly authorized by my daughter and son-in-law,


_________________ and_______________, to hold and possess the
certificates of title of their real properties, specifically TCT
Nos.________________;

2. That while I was in the Register of Deeds of Trece Martires, Cavite


on June 28, 2019, I noticed that the said titles in my possession
were missing;

3. That prior to this, I placed said titles in a brown envelope which I


carried while I was on my way to the Register of Deeds of Trece
Martires, Cavite;

4. That upon learning that the brown envelope with titles were
missing, I returned to the areas I passed-by, but unfortunately, I
did not find those;

5. That I asked the people in the areas I passed-by if they happened


to see a brown envelope with titles, but they said they did not find
any;

6. That despite diligent and earnest efforts, I cannot locate the


missing brown envelope with titles;

7. That I am executing this affidavit to attest the truthfulness of the


foregoing declarations under penalties of perjury, for whatever
legal purposes this may serve.

FURTHER AFFIANT SAYETH NAUGHT.
