(address niya) do hereby appoint, name, & constitute, MARIANO P. FLORES,
Filipino, of legal age, single, and with postal address at 1418 Penafrancia St., Brgy.
682 Zone 74, Paco Manila, to be my true and lawful attorney-in-fact, for me and my
name, place, and stead, to do and perform the following acts, to wit:

1. To follow-up the status of (yung title ng case at case number);

2. To RECEIVE any order, resolution, and/or decision attached on the record of


the aforementioned cae;

3. To make, sign, execute and deliver any and all papers, documents writings or
instruments of whatever nature or form which are necessary or essential to
comply with the above-mentioned authority or in any way connected
therewith;

HEREBY GRANTING AND GIVING unto our said attorney full powers and
authority to do and perform all and every act necessary to carry into effect the foregoing
authority as fully to all intents and purposes as I might or could lawfully do if personally
present, and hereby ratifying and confirming all that my said Attorney or her substitute
