DECLARATION OF TR UST

I, ____________________, of legal age, with address at ________________________, after


having been duly sworn in accordance with law, DO HEREBY declare as under:

That I am the holder of a total of ___________ (_______) shares of the capital stock of
____________________________________________ (the “Corporation”) in trust for, or for the
benefit of, ___________________________ ("Trustor");

That the herein Trustor now wants to fully exercise its right over the above-mentioned
shares;

That by virtue of this declaration, any and all rights pertaining to the above-mentioned
share shall now be vested in the Trustor or a new Trustee it may designate and that we do
hereby request and direct the Corporation or its authorized Transfer Agent to cancel the herein
attached certificates of stock issued in our names as Trustee and therefore issue a new certificate
in the name of the Trustor herein, or whoever is named as the new Trustee.
