AFFIDAVIT
I, TEODORO, JR. TALAUE ZIPAGANG, of legal age, married, Filipino
citizen and a resident of Centro, Sto. Tomas, Isabela after having been duly
sworn to in accordance with law, do hereby depose and state THAT:
1. I was elected and proclaimed as Sanguniang Bayan of Sto. Tomas,
Isabela last May 10, 2010 National aand Local Elections;
2. I received a written communication coming from Atty. Ferdinand T.
Rafanan, Director IV of the Campaign Finance Unit, stating therein
my failure to submit my Statement of Contribution and
Expenditures (SOCE);
3. I have submitted my statement of Contribution and expenditures
(SOCE) sometime on the first week of June 2010 before the Office
of the election Officer of Sto. Tomas, Isabela, later, informed me of
its submission before the LAW Department last June 25, 2010;
4. I decided to entrust my file copy of my SOCE with the Office of the
Election Officer, Sto. Tomas, Isabela;
5. Said file was destroyed during October 16, 2010 super typhoon
Juan (Signal No. 4) and followed by typhoon Kiel, where the
Municipal Hall that housed the Office of the Election Officer was
flooded and rendered roofless;
6. I am executing this affidavit to state the truth and the fact that I
have submitted within the thirty (30) day period, my Statement of
Contribution and Expenditures (SOCE) in accordance with Section
14 of republic Act 7166 and therefore, the affiant respectfully
believe that he/she is not to be fined nor suffer any legal sanctions
as provided for in Section 14 of Republic Act No. 7166 and other
appropriate Election Laws and resolutions.

AFFIDAVIT OF EXPLANATION
I, EDY MIGUEL MESA, of legal age, married, Filipino senior citizen and a resident of Centro,
Cabagan, Isabela after having been duly sworn to in accordance with law, do hereby depose and state
THAT:
1. That on November 26, 2020, I received a letter from the Campaign Finance Unit of
Commission on Elections (COMELEC), Manila requiring me to submit within fifteen (15) days
from receipt thereof a written explanation under oath stating the reasons for failure to file the
Statement of Contributions and Expenditures (SOCE) and why no fine should be imposed
upon me;
2. That this Affidavit of Explanation is submitted within the period set by law;
3. That I am one and the same EDY MIGUEL MESA who ran for City Counselor in City of
Ilagan, Isabela in the elections of May 2016;
4. That with all due respect to the Honorable Commission on Elections especially the Campaign
Finance Unit, I am executing this Affidavit of Explanation to explain my failure to comply with
the requirement of the law on Statement of Contributions and Expenditures;
5. That I failed to submit my SOCE because I was completely not knowledgeable that even the
candidates who lost in the election are required by law to submit Statement of Contributions
and Expenditures;
6. That it was never my deliberate intent to disregard the requirement of the law if I had known
immediately;
7. That the failure to submit such document was due to honest mistake and negligence;
8. That I am willing to comply with the said provision of the law but I pray that I be given
sufficient time to find and recall my records on the said 2016 election because it was more
than four years ago;
9. That I am praying to the Honorable Commissioners of the COMELEC thru the Head of the
Campaign Finance Unit that no fine shall be imposed upon me because of aforementioned
circumstances.
10. I am executing this affidavit to state the truth of all the foregoing facts in compliance with the
Letter of the head of the Campaign Finance Unit.
