checklist before submission of data


Line Each (Missing /Extra)
Improper Alignment left indent bottom alignment
Spelling Error check in occupation and remarks, status etc
Word capital Every sentence
Space replace each double space with single
Punctuation remove , (comma) from address
Sufix field put fullstop and leave space
zip code make capital remove double space
mail I'd make all letters small, remove all spaces
address field separate each field, separate south-north-east-west
doctor's name field check Dr. (space) everywhere
name field first coloumn dot space
second coloumn dot space
third coloumn only dot
replace hyfen with dot
birth date field remove space and dd mmm yyyy format
remark field check spelling mistakes, capital PCP, TSH, OTC etc.
disease field check known spellings (e. g. disease, syndrome etc.)
occupation field check known spellings (e. g. Technician, carpenter, driver etc.)
phone number field remove all spaces, put brackets to code e. g. (44)
