         I, MARIO B. DELA CRUZ, of legal age, Filipino, single and a resident
of 232 Street, Biday, San Fernando City, La Union, after having been duly
sworn to in accordance with law, depose and state:

1. That I am employed as a messenger of Atty. Rossette S. Anasario with office


address at CDE LAW FIRM, 3RD FLOOR, SAN FERNANDO CITY, LA UNION;

2. That on November 20, 2019, I served a copy of the following pleading/paper


by registered mail in accordance with Sec. 10 of Rule 13 of the Rules of
Court:
