I, JERLYN V. ROY , of legal age, Filipino, and with address at 2372 VELASCO
AVENUE, NAPICO, MANGGAHAN, PASIG CITY, METRO MANILA, NCR , after
having been duly sworn in accordance with law, hereby depose and state that:

1. I am the STEP MOTHER and the Guardian-in-fact of VESLINO, SOFIA ABBY J.


, who is currently a GRADE 8- SARANGANI student at STA.LUCIA HIGH
SCHOOL.

2. As STEP MOTHER, and in the absence of his/her parents, I legally exercise


substitute parental authority over him/her, in accordance with Article 216 of the
Family Code. As such, the Child is under my care, custody, and support.

3. I am executing this affidavit in order to attest to the truth of the foregoing and in
compliance with the requirements of the Government of Pasig in claiming the
educational gadgets/equipment for the said Child.
