Discharge Summary for Mr. Ramamurty

Mr. Lionel Ramamurty was admitted to the hospital due to pneumonia. He has since stabilized his breathing and can walk slowly without assistance, use the toilet, and shower. Re: Lionel Ramamurty

Mr. Ramamurty, who was admitted due to Pneumonia. He is now ready for discharge back into your
care.

Upon admission, he has difficulty of breathing, persistent coughing and wheezing upon auscultation. He
also had fever and generally body weakness.

Mr. Ramamurthy has stabilized his breathing status. He has able to walked slowly without assistance,
and used of toilet and shower. However, he has some chest and abdominal pain during cough.
Paracetamol was prescribed if necessary, for his pain. He has encouraged to do deep breathing and
coughing exercise. He prefer to be maintained at sitting position while lying down for postural drainage.

For his recovery, he requires good nutrition that includes egg, fruits, and vegetables on his diet and he
needs diet monitoring. Kindly encourage him to increased oral fluid intake for his hydration, and keep
him warm.
