Undesired events outside the definition of incident (For critical
equipment failure follow HSQEEn 05.04)
Undesired events include:
• Property damage e.g.

- Heavy weather damage
- Third party property damage not falling under incident

• Commercial delays not falling under incident category
• Cyber attack
• Machinery failures with losses equal to or more than

Investigation and follow up procedure:
• The vessel shall capture above undesired events as
CARs (type: S-Undesired Event) in NS Enterprise
• The vessel shall correct the CAR after completing the
investigation.
• The vessel superintendent shall verify the CAR and
capture identified Corrective Actions as pending defect
• Cyber attack
• Machinery failures with losses equal to or more than
10000 $
Investigation and follow up procedure:
• The vessel shall capture above undesired events as
CARs (type: S-Undesired Event) in NS Enterprise
• The vessel shall correct the CAR after completing the
investigation.
• The vessel superintendent shall verify the CAR and
capture identified Corrective Actions as pending defect
WO's within the CAR.
• The vessel / vessel superintendent shall complete the
pending defect WO’s as applicable.
• The fleet manager shall close the CAR after verifying
closure of all pending defect WO’s.
Notes:
1. The vessel superintendent shall prepare simplified
investigation report in case we have to share the
investigation report with the external parties. FST to
review the report if it has to be shared with an oil major.
2. Machinery failures with losses < 10000 $ to be
captured as Failure WO's in vessel PMS.
