SUBJECT: URGENT SUBMISSION OF VITAL INFORMATION ON FIRE PREVENTION AND
SUPPRESSION ACCOMLISHMENTS REQUIRED BY DBM AND AO25 SECRETARIAT
FOR THE EVALUATION OF BFP CY 2014 ACCOMPLISHMENT REPORT FOR THE
GRANT OFPERFORMANCE- BASED BONUS (PBB)

With the prescription by DBM of the redefined Major Final Outputs (MFO) and Performance Indicators
(PIs) of the BFP for CY 2014, you are hereby directed to include in your periodic reports on the relevant fire
prevention and suppression statistics the following data/ information (See MFO 1 and 2) covering the period of
January- September 2014:
1. RESPONSE TIME ( from the time the fire call is received until such time the first responding
firetruck arrives at the fire scene). The periodic statiscal report should be able to indicate the Average
Response Time on a City/ Municipal, Provincial/ District and Regional level.
2. Number of Fire Incidents involving structures ISSUED WITH FIRE SAFETY INSPECTION
CERTIFICATES (FSIC); The periodic statiscal report should be able to indicate the percentage share
or absolute figures of FSIC- rated fire incidents on a City/ Municipal, Provincial/ District and Regional
levels and;
3. Number of Fire Incidents with LOW LEVEL OF ALARM (3rd Alarm and Lower). The perodic
statiscal report should be able to indicate the percentage share or absoluter figures of number of fire
incidents with low level of alarm on a City/ Municipal, Provincial/ District and Regional levels.
4. DURATION IN THE CONDUCT OF FIRE SAFETY INSPECTION. All fire stations/
substations should be able to track down the duration or time spent by fire safety inspectors to conduct
of fire safety inspection from the time they arec assured with Mission Orderts. The periodic statiscal
report should be able to present the average inspection duration on a City/ Municipal, Provincial/
District and Regional levels.
The aforesaid information is very vital for evaluation of this year’s BFP Accomplishments by the AO25
Secretariat relative to the grant of Performance Based Bonus (PBB) CY 2014. For your ready reference, we are
providing the DBM- Approved Organizational Performance Indicators Framework (OPIF) Logframe and
the CY 2014 Physical Plan (BED Form 1)by sending the hard and soft copy through fax and in your official
email account respectively.
Submit your properly accomplishs oft copies of MFO Forms 1 and 2 matrices through sending it in
operationisabelaro2@yahoo.com not later than November 30, 2014 for consolidation.

MFO FORM 1
MFO 1-FIRE PREVENTION SERVICES
This form reflects over-all profile in fire on fire or safety inspection conducted by the city municipal fire station in
the city/municipal level. All reports accomplished at the fire station/sub-station shall be consolidated at the
city/municipal Central Fire Station and thereafter consolidated at the Provincial/District and Regional levels. Hard copies
of the reports from each city/municipal fire station shall be kept intact for purposes of operation performances
audit/audit validation.
Guidelines in Filling-up the Form in Summary of Accomplishments in the
Conduct of Fire Safety Inspection
Column 1.Name of the Affected Building/establishment. Specify the name of the establishment
subjected to fire safety inspection. Establishments include buildings/structures where there are
various establishments in one(1) building, locator establishments are counted separately and
independently from the building. The mall/building for example, is counted as an additional one
establishment.
2. Location/Address. The street location of the establishment to include building/street number,
street name and the barangay.
3. Name/Rank of Assigned Fire Safety Inspectors. Give the name and rank of the designated Fire
Safety Inspector (FSI) assigned to conduct the inspection in that particular establishment.
Identify only the name and rank of the Team Leader in the Fire Safety Inspectors is composed of a
team and only the name /rank of the assigned Fire Safety Inspector if an individual is involved.
4. Nature of Inspection. Specify whether the said inspection was conducted for the first time
(initial inspection) or re-inspection.
5. Date/Time of Inspection Order (IO) Issued. Date/Time when the duty signed Inspection Order
(I.O) was received by the assigned Fire Safety Inspector.
6. Date/Time of Submission of After Inspection Report. (AIR). Date/Time of Submission of after
Inspection Report (AIR) after the conduct of Fire Safety Inspection has been completed.
7. Duration of the conduct of Fire Safety Inspection (Days/Hours).The difference (in days/hours)
from the time inspection order received by FSI until such time when the after Inspection Report
was received by the City/ Municipal fire Marshal.
8. Remarks. Provide additional information relative to the conduct of Fire Safety Inspection in the
particular establishment.

MFO 2-FIRE SUPPRESION AND INVESTIGATION SERVICES
This form gives the summary of individual fire incidents as described in the Spot Investigation Report (SIR)
and/or After Fire Operation Report (AFOR). All reports accomplished at the fire station/sub-station shall be
consolidate at the City /Municipal Central Fire Station and thereafter consolidated at the Provincial/District
and Regional levels. Hard copies of the reports from each city/municipal fire station shall be kept intact for
purpose of operations performance audit/validation.
Guidelines in Filling-up the Forms in Summary of Accomplishments in the Conduct of Fire Suppression
Activities by the First Responding Unit
Column 1.Date.Date when the fire incident took place
2.Location. Location of the fire incident to specify the building no., street, address and
barangay where the fire occurred.
3.Departure Time. Time when the fire truck from the first responding unit departs from the station.
This column shall be in military time format (2400H).
4.ArrivalTime. Arrival time of the fire truck from the responding unit in the fire scene.
This column shall be in military time format (2400H).
5. Response Time. The differences (in minutes) between the arrival time (D) and the time when the
firetruck departed from the fire station, i.e. 5 = 4 – 3. It is the time that begins when the first
responding unit are end route to and ends upon its arrival at the fire scene. This column shall be in
military time format (2400h).
6. Approximate Distance (in Kms.) of the Nearest Fire Station. Provide information on the
approximate distance from the scene of the nearest fire station from which the first responding
units came from.
7. Occupancy Type of Fire Origin. Type of occupancy of the involved building /structure identified
as the fire origin.
8. Name of the Affected Building/Establishment. Specify the name of the building /establishment
(if applicable) identified as the fire origin.
9. Number of Storeys. Number of stories of the building where the fire originated.
10.Issued with FSIC. This column indicates whether or not the building/establishment identified as
the fire origin has been issued with Fire Safety Inspection Certificate (FSIC) prior to the fire incident.
11. Estimated Amount of Property Damage. Give an estimate of the amount of property damage
but the fire incident from the time it was reported until such time the said incident was declared
“Fire Out”.
12. Highest Alarm Reached. The highest alarm level reached for that particular fire incident.
13. Casualty Report. The casualty report includes the number of organic BFP personnel who were
Killed/injured as well as civilians, including fire brigade volunteers injured/ killed in said incident.
14. Remarks. This columns provides additional information such as the number of employed
individuals who lost their job as a consequence of the fire that affected their work place as well as
the number of households rendered homeless by the fire incident.
