Kauswagan, Lanao del Norte, after having been subscribed to an oath, depose and say:

1. That I am about to enter into a contract of marriage with my Filipina fiancée;


2. That under Philippine laws on marriage, a birth certificate is among the requirements;
3. That my birth certificate is expressed in Chinese language;
4. That based on my birth certificate, the following are true and accurate personal
circumstances to the best of my knowledge:
