SWORN STATEMENT

I, KRYSTEL JOY B. TEMARIO-MATONDO, Filipino, of legal age, married, and a


resident of Tagoloan, Misamis Oriental, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the direct granddaughter of SERGIO BUMAYA TEMARIO, a member of


the USAFFE ( United States Army Forces in the Far East);

2. That the papers and documents that would serve as my evidence to support my claim that
the late SERGIO BUMAYA TEMARIO was a USAFFE member was lost;

3. That the aforementioned papers and documents were under the custody of my aunt, my
father’s sister, LEONIDA TEMARIO ABELLA, a resident of Iponan, Cagayan de Oro
City, which unfortunately, due to a destructive typhoon “Sendong” last December 13,
2011, it was destroyed and lost;

4. That despite diligent efforts to recover the same, I failed to locate the said papers and
documents;

5. That I am executing this affidavit to attest to the foregoing facts.

In witness whereof, I have hereunto set my hand this 26th day of November 2019 at Cagayan
de Oro City, Philippines.
