NOTICE OF COORDINATION
(With Acceptance Certificate)
To Whom It May Concern,

This is to ACCEPT and ALLOW ENTRY of CLARITA MONTILLA MATURAN of legal age, Filipino,
male/female who wants to come to Barangay TALAMBAN, CEBU CITY from where he/she was
stranded due to community quarantine. Provided, that upon his/her arrival to this barangay, he/she
shall comply with the following terms and conditions:

1. He/She shall proceed directly to the barangay health center and present the original
medical certificate issued by the Municipal/City Health Office stating that he/she is
neither a contact, suspect, probable nor a confirmed COVID-19 case in the LGU of origin;

2. He/She shall undergo swab test to be scheduled by the authorized personnel of the
barangay health center;

3. He/She shall complete the 14-day quarantine in his/her home if separate isolation room
and comfort room are available. Otherwise, he/she shall be placed at the designated
isolation center.

This certification is issued upon his/her request for TRAVEL PURPOSES and for whatever legal
purpose this may serve.

Issued this 3rd day of November 2020 at Barangay Talamban, Cebu City.
