JUDICIAL AFFIDAVIT OF RICHIELOU B. CRUZ
I, RICHIELOU B. CRUZ, Filipino, of legal age, single, and with
the residential address at 2003 Manila Executive Regency Jorge
Bocobo St., Ermita, Manila, after having been duly sworn to, in
accordance with law, hereby depose and state:
The person examining me is Atty. Isnihayah M. Pangandaman,
with the office address at 1830 Manila Point Plaza, Ermita, Manila. The
examination is being done at the same office.
That I am answering her questions fully aware that I do so under
oath and that I may be criminally liable for false testimony and perjury
if I tell lies or misrepresent.

Q2. Please state your personal circumstances for the record.
A2. I am Richielou B. Cruz, 30 years old, dentist and residing in
Ermita, Manila.
Q3. Are you the same person who filed the Complaint-Affidavit dated
October 5, 2019?
A3. Yes ma’am.
Q4. Do you know a certain Rose M. Vega?
A4. Yes ma’am.
Q5. How did you come to know Rose M. Vega?
A5. We used to be block mates in UST.
Q6. I am showing you this document, herein referred to as Annex “A”.
Do you know this document?
A6. Yes ma’am.
Q7. What is this document?
A7. It is a promissory note.
Q8. Who executed this promissory note?
A8. Rose M. Vega ma’am.

Q9. To whom the promissory note was payable?
A9. To me ma’am. I lend the P5,000,000 to Rose M. Vega ma’am.
Q10. When was this promissory note executed by Ms. Vega?
A10. It was on June 1, 2018 when she went to my house to lend
money.
Q11. When does the promissory note become due and demandable?
A11. One year after or June 1, 2019.
Q12. After the promissory note became due and demandable, what did
you do?
A12. I asked her to pay and sent a demand letter to Rose M. Vega.
Q13. I am presenting this document herein referred as Annex “B”. Do
you know this document?
A13. Yes. That is the first demand letter I sent to Rose Vega.
Q14. Was there any payment made by Ms. Vega in response to the
demand?
A14. No, ma’am. She asked me to giver her two more weeks for her to
pay.
Q15. What happened after that?
A15. I waited for two weeks. And then I sent her another demand
letter.
Q16. I present to you this document herein referred as Annex “C”. Do
you know this document?

A16. Yes, it is the second demand letter received by Rose Vega from
me.
Q17. Was there any payment made?
A17. No ma’am.
Q18. Did Rose Vega made any attempt to pay?
A18. No ma’am.

I, ATTY. ISNIHAYAH M. PANGANDAMAN, with office address at
1830 Manila Point Plaza, Ermita, Manila, attest under oath as follow:
That I personally conducted, supervised and recorded the examination
of Richielou B. Cruz, re: “Richielou B. Cruz vs. Rose M. Vega” docketed
as Civil Case No. 21808, For: “Collection of Sum of Money” pending
before the Regional Trial Court of City of Manila, Branch 15. That I
faithfully recorded the questions and the corresponding answers given
by him and neither I nor any person present coached or influenced
Richielou B. Cruz in his answers.

evidence of identity and likewise made an oath that she is fully aware
that she is being examined under oath and is fully aware that she may
be criminally liable for false testimony or perjury if she tell lies or
make misrepresentation and Atty. Isnihayah M. Pangandaman, the
lawyer who conducted and supervised the examination and caused the
recording of the questions and answers given thereat who presented
to me a valid identification card as competent evidence of identity,
which bears his picture and signature thereon. Basing on the
identification cards, the pictures and signatures appearing thereon, the
affiant and the lawyer, are one and the same persons who appeared
before me and whose signatures in this document are the same as
their signatures thereon.
