AFFIDAVIT OF NON-COHABITATION
I, MA. RHEA B. ALVAREZ, of legal age, Filipino, single and a resident of
Labrador Compound Sineguelasan Bacoor, Cavite after having been duly sworn to
in accordance with law, depose and state:
1. I am currently married to Joshua E. Ramos;

2. However, the above-stated person and I have been separated for almost
2 years and both of us have no plans to reconcile neither now in the
near future;

3. That we have separated by reason of irreparable differences;

4. I am aware that my husband is also living in the same compound;

5. Thus, I attest under oath that I am not living with my husband ever
since we separated from each other;

6. That it is known to the knowledge of our neighbors that my husband


and I are no longer cohabiting;

7. The purpose of this Affidavit is to affirm the facts alleged herein and
for any legal purpose it may serve.
