1. Reference: Memo received from MIS OR PPO dated January 8, 2020


with same subject as above.

2. In connection with the above reference, I do hereby submit this explanation


letter in response to a memorandum for failure to comply/submit accomplishment on
PNP Campaign Plan: “Clean Rider” Monitoring for the period of December 24-30,
2019 and December 31, 2019 – January 6, 2020;

3. That, we’ve had no Internet connection from December 28, 2019 as


PLDT internet connection within the municipality of Initao was down per Blotter entry
No. 2019-12-13008;

4. That, the undersigned have complied said directives however, because of


no internet connection in our AOR, we were not able to email our reports/compliances
on time;

5. That, PLDT internet connection was only fixed on January 2, 2020 per
Blotter entry No. 2020 – 01- 1315 hence said compliances including all other reports
from the different divisions were only emailed since then;

6. That, the undersigned have always rendered my best effort to the PNP
organization and have been conscientious in the performance of my duties;

7. That, I always show extreme diligence at work however unintentionally


incurred mistake;

8. That the undersigned will take necessary steps to ensure that this offense
is prevented in the future;
9. That, the foregoing is true and correct and prays that this explanation be
acceptable and given merit;

10. Other relief, just and equitable are also prayed for.
