in the account name of “SANGGUNIANG KABATAAN OF BARANGAY DINGANEN”.
In view hereof, we are hereby submitting to your bank the following documents for your
ready reference:
1) Certificate of Recognition from DILG Regional Office;
2) Certificate of Canvass (COC) and Proclamation from COMELEC;
3) SK Resolution No. 2018-002, s. 2018 confirming the appointment of SK Treasurer
RICHARD IVAN G. BONGHAYAG;
4) SK Resolution No. 03, s. 2019, authorizing the SK Chairman and SK Treasurer to open
account and to encash/disburse funds of the SK;
5) 1st and 2nd Indorsements from the DILG;
6) Fidelity Bond from the Bureau of Treasury;
7) Php 10,000.00 for cash deposit to this Sangguniang Kabataan account to be opened;
8) Oaths of Office of:
a. The SK Chairman
b. The SK Treasurer
9) Two (2) valid IDs;
10)Four (4) pieces (1x1) colored picture; and,
11)Tax Identification Number (TIN).

A RESOLUTION CONFIRMING THE APPOINTMENT OF RICHARD IVAN G. BONGHAYAG AS
SANGGUNIANG KABATAAN TREASURER, BARANGAY DINGANEN, BULDON, MAGUINDANAO.
Whereas, presented to this august body for serious consideration, approval and
confirmation, is the proposed appointment papers, and its supporting documents, of RICHARD
IVAN G. BONGHAYAG as SK Treasurer effective July 9, 2018;
Whereas, upon scrutiny, review and assessment of the same documents and the
qualifications of the proposed appointee by this Sanggunian, the same have been found in order and
the appointee, qualified for the position.
Wherefore, on motion of SK Kag. JEAN M. GALILEA, duly seconded en masse, be it
Resolved, as this Sanggunian does hereby resolve, to approve and confirm the appointment
of RICHARD IVAN G. BONGHAYAG as Sangguniang Kabataan Treasurer, this barangay, as it is
hereby approved and confirmed.

A RESOLUTION AUTHORIZING THE SANGGUNIANG KABATAAN OF BARANGAY DINGANEN TO
OPEN SAVINGS ACCOUNT AT THE DEVELOPMENT BANK OF THE PHILIPPINES (DBP), WITH
SK CHAIRWOMAN EPIPHANY V. TUCAS AND SK TREASURER RICHARD IVAN G. BONGHAYAG,
AS THE OFFICIAL SIGNATORIES THEREOF AND AUTHORIZING THEM FURTHER TO
ENCASH/DISBURSE FUNDS OF THE SANGGUNIANG KABATAAN.
WHEREAS, there is a need for the Sangguniang Kabataan of Barangay DINGANEN, Buldon,
Maguindanao, to have a savings account at the Development Bank of the Philippines, Cotabato City;
WHEREFORE, on motion of Kag. JEAN M. GALILEA, duly seconded en masse, be it
RESOLVED, as it is hereby resolved, to authorize the SANGGUNIANG KABATAAN OF
BARANGAY DINGANEN, Barangay Dinganen, Buldon, Maguindanao, to open a Savings Account at
the Development Bank of the Philippines (DBP), Cotabato City, with SK Chairwoman EPIPHANY V.
TUCAS and SK Treasurer RICHARD IVAN G. BONGHAYAG, as the official signatories thereof;
RESOLVED FURTHER, to grant authority to SK Chairwoman EPIPHANY V. TUCAS and SK
Treasurer RICHARD IVAN G. BONGHAYAG, as they are hereby granted authority, to
encash/disburse funds of the Sangguniang Kabataan in said DBP bank account to be opened;
RESOLVED FINALLY, that this resolution along with other pertinent documents be
forwarded to the Development Bank of the Philippines (DBP), Cotabato City, for their information
and appropriate action.
