JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ADELAIDA G. LORENZANA and SUSANA D. MANITO, both of legal age,


Filipinos, and residents of Barangay Valencia, Ormoc City, Leyte, Philippines, after having been
duly sworn to in accordance with law, do hereby depose and say, that:

1. We personally know EDUARDO BARBOSA CODOG who was born on January


04, 1951 in Ormoc City, Leyte, Philippines by the spouses CRISANTA BARBOSA-
CODOG and CLEMENTE A. CODOG (as per Certificate of Birth under Registry
No. 97-5447) and our longtime neighbor and acquaintance in Barangay Valencia,
Ormoc City, Leyte;

2. The subject person is a bonafide member of SSS and issued a corresponding SSS
number 06-0380930-7;

3. Appearing in the record of SSS, his date of birth was erroneously entered as January
04, 1950 instead of January 04, 1951;

4. Further, upon the record of SSS, he has no middle which is CODOG;

5. That the said discrepancy was purely due to inadvertence the same and for the lack of
supporting documents upon complying the requirements hereinafter due to late
registration and legitimation by subsequent marriage of his parents;

6. We are not in way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have pecuniary interest in this
matter whatsoever;

7. We hereby declares under oath that the person EDUARDO BARBOSA CODOG
who was born on January 04, 1951 (per Certificate of Live Birth) and the person
name EDUARDO CODOG who was born on January 04, 1950 (per SSS record)
refer to one and the same person and that his true correct name and date is
EDUARDO BARBOSA CODOG born on January 04, 1951;

8. We are executing this affidavit to attest the veracity of the foregoing facts and for
whatever other legal purpose this serves best.
