CITIZENS MEMORIAL HOSPITAL HOME CARE SERVICES, BOLIVAR, MISSOURI
In this position I provide evidence-based physical therapy treatments to rural patients
throughout eight surrounding counties. The patients treated have a wide variety of impairments
as well as recent total hip arthroplasty or total knee arthroplasty. Patients are provided with
personalized home exercise programs and educational materials regarding their diagnosis or
surgical procedure. I have provided two in-services for staff members to educate on the benefits
of aerobic exercise following stroke as well as objective pain measurements and non-
pharmacological methods for pain reduction. This position requires strong organizational and
time management skills for scheduling and completion of all home visits. I have been recognized
by several patients through positive surveys and cards to the facility as providing outstanding
care with excellent results.

back safety and Hoyer transfer training to new hospital clinical staff.
