AFFIDAVIT OF DISCREPANCY

I, CECIL MANDAP ARCINUE, of legal age, Filipino, married to Roberto Loreto Arcinue,
and a resident of ________________ City, Philippines, having been duly sworn in
accordance with law, hereby depose and state:

That in the Philippine Statistics Authority copy of my Marriage Contract as recorded in the
Office of the Civil Registrar of Tuba, Benguet, Philippines, it is stated that I was born on 29
April 1955;

That in the Philippine Statistics Authority copy of my Certificate of Live Birth as recorded in
the Office of the Civil Registrar of Capas, Tarlac, Philippines, under my maiden name Cecil
Pascacio Mandap, it is stated that I was born on 03 March 1955;

That in my all other identification documents, such as, my Philippine passport __________
issued by DFA Baguio expiring on 25 May 2029; my Senior Citizen ID ________ issued by
OSCA Baguio City; and my SSS ID __________, all state that my birthdate is 03 March
1955;

That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing facts
and to explain the discrepancy in my Marriage Contract and my Birth Certificate, as regards
to my date of birth;

Further, I attest that my true and correct date of birth is 03 March 1955 as evidenced in all
my papers, records and other documents.
