Leo Jun I. Mauricio is a 41-year-old member of the United Employee Multipurpose Cooperative located at South Cotabato State College who has taken out a loan. As a cooperative member, his loan was paid through salary deductions but he has now been promoted and transferred to a new job that will end the salary deductions. He promises that once he receives his new ATM card from his new employer, he will voluntarily make deposits to repay his loan obligation to the cooperative.

PROMISSORY NOTE

I, LEO JUN I MAURICIO, 41 years old and a residence of Brgy Dajay, Surallah South Cotabato and official
member of the United Employee Multipurpose Cooperative located at South Cotabato State College
Campus, Surallah South Cotabato.

The UEMPC is a close type cooperative that provide lending business to an employee of the SCSC and
SNHS. As a member, I have an obligatory to the cooperative through salary deduction.

At this event I was promoted and transferred to the PTC-Surallah which will cause as to delist on the
payroll deduction.

I promise that as soon as I receive my new ATM Card from PTC- Surallah, I will voluntarily deposit to the
UEMPC to supplement my obligation. I affix my signature as to conform this promissory note.
