AFFIDAVIT OF LOSS
I, Rico B. Geron (Sorosoro Ibaba Development Cooperative), 55, Filipino, married and a
resident of Barangay Alangilan, Batangas City, Philippines, after having been duly sworn in
accordance with law, hereby deposes and state:

1. That I am the Chief Executive Officer of Sorosoro Ibaba Development Cooperative


(SIDC) located at Sorosoro Ibaba, Batangas City with a Registered Business: Sorosoro
Springs Resort and Adventure Park;

2. That the Trade Name Sorosoro Springs Resort and Adventure Park under Sorosoro Ibaba
Development Cooperative was duly registered with the Bureau of Internal Revenue (BIR)
with Tax Identification No.000-639-016-000 and was therefore issued a BIR
CERTIFICATE OF REGISTRATION (BIR Form 2303) with OCN 1RC00007329;

3. That unfortunately, the original copy of BIR Certificate of Registration of Sorosoro


Springs Resort and Adventure Park issued by the BIR was lost and I took pains to look
for the original copy of said BIR Certificate of Registration but to no avail;

4. That despite diligent efforts exerted, I could no longer locate, find or recover the issued
Certificate of Registration, and to the best of my knowledge, the same is truly lost;

5. That said Certificate of Registration has not been confiscated by any officer of the law or
the issuing office or agency due to the violation of any law, statute, order, rule or
regulation;

6. That in the event that the lost Certificate of Registration should hereafter be found or
located, I undertake to forward or surrender the same to Bureau of Internal Revenue
(BIR); and

7. That I am executing this affidavit to attest to the truth of the foregoing statement and to
request for the issuance of a new and original Certificate of Registration with OCN
1RC00007329 in lieu of the one which was lost.
