AFFIDAVIT AS TO NAMES

I, GEOFFREY KOBERO, adult, Christian of P. O. Box 96224 Dar es Salaam do hereby


make oath and state as follows:

That I am the Administrator of the Estate of the late SARAH. C.B. KOBERO vide
probate and Administration cause No. 38/2008 High Court of Tanzania.

That the deceased died on 20th MARCH 2008 at Mission Mikocheni Hospital, Dar es
Salaam.

That there was typing error of the deceased names in the death Certificate, which reads
as SARA KOBELO instead of the proper names which are SARAH C.B. KOBERO.

That I swear this affidavit to the effect that the proper names are SARAH C.B.
KOBERO as it appears in the Letter of Administration..
