JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, MARK T. LALLEN , of legal age Filipino, and presently residing at 3845 Gumamela
St., Napico Manggahan, Pasig City, and CHERRY CABALLERO LALLEN, also of legal age, Filipino
and residing at 3845 Gumamela St., Napico Manggahan, Pasig City, after having been duly
sworn in accordance with law, hereby depose and say:

1. That we personally know the late JIARY MATORE EUSEBIO SR. who died on August
15, 2020 as per his Death Certificate which is hereto attached;

2. That during his life-time he used the name JIARY MATORE EUSEBIO SR. later on his
name known as JIARY MATURI EUSEBIO which is true and correct name as per his
Certificate of Birth ;

3. That we also know and found out that his name inadvertently written with the
Certificate of Live Birth of his children to wit:
Name Discrepancies of their father’s name
Liezl F. Eusebio and Noel F. Eusebio DIARY MATORE EUSEBIO
Jacqueline F. Eusebio JARRY EUSEBIO
Jiary F. Eusebio Jr. JIARY MATORE EUSEBIO SR.
Rommel F. Eusebio JIARY M. EUSEBIO
All names reflected with the Certificate of their children refer to One and the
same
Person of the deceased person.

4. That we also know there are discrepancies regarding the name of mother of JIARY in
which:
Document of the deceased Discrepancy of his mother’s name
Birth Certificate Henrita Gaa Maturi
Marriage Certificate Hindry Matore
Death Certificate Hendry Matore
All refer to as one and the same person of the deceased mother

5. That we know the foregoing facts having been a longtime Neighbor and closed-
friends of JIARY MATORE EUSEBIO SR. later known as JIARY MATURI EUSEBIO as
his Correct and true correct name as per his Certificate of Birth and we are not
related either by affinity or consanguinity and we are not in any manner interested
on the result for which this affidavit is being executed;

6. That we are executing this affidavit to attest to the truth of the foregoing statement
and for whatever purpose this may serve.
